You really have to read the book (
Dr. Atkins' New Diet Revolution, 2002) to learn how to do Atkins correctly. There are also sticky threads on top of the OWL forum, which you should read. It is totally expected for Atkins not to work when you are not doing it as it's supposed to be done.

If you increase your carbs during OWL, you have to increase them by maximum 5 g of net carbohydrate per week. So in the second week you could have had maximum 25 net carbs, while in the fourth week you can have maximum 30. However, the 5 g that have to be added should be added to the average number of net carbohydrates you had been eating in Induction. If you had only 3 cups of vegetables and over half of your net carbs came from vegetables (as the book says it should) then your net carbs were below 20.
It is also preferable to either introduce a new food per week or to increase your carbs, but not both. Otherwise you will have problems figuring out what stalled you -- the new food or the extra carbs.

I'm not sure why you think Crystal Light is superior to diet sodas, but it isn't. It can stall you just the same since they are both sweetened with aspartame and contain a bunch of unpronounceable ingredients.
You should also consume no more than 3 servings of sweetened food/drink per day. For drinks, a serving is usually 8 oz.

It depends on the ingredients in the bread. If the ingredients are allowed during the OWL Rung you're on, then the bread is "legal". 9 g of net carbohydrate per slice sounds a lot to me -- since new foods have to be added in 5 net carbs amounts, you'd have to limit yourself to 1/2 slice per day. The Nature Own's bread you are talking about is probably the Sugar Free 100% Whole Grain
Wheat that is not allowed before OWL Rung 9, because it contains wheat. By the way, Nature Own's Double Fiber Wheat is lower in net carbs (5 g per slice), although it is still not allowed before OWL Rung 9.
Peanuts can be introduced during OWL Rung 3. Check the book to see in what quantity you can have them.

Whether you are in OWL or not doesn't really depend on how much weight you've lost, but on the foods and net carbs you are consuming.
A rung is a step on a ladder, so on Atkins, a Rung is a "step" on the Carbohydrate Ladder. The Carbohydrate Ladder is something Dr. Atkins talks about in
DANDR, so go read the book to understand what we're talking about.
